I have an outline of sorts, in the sense that I know, or think I know where the story goes. Otherwise, I do an outline for the immediate sections ahead. As soon as I have an outline--I get writer's block. So, in reality, in spite of my best intetions, it's a very sketchy one and it's mostly in my head.

Amazing, how each one of us 'creates' differently. :) I am always impressed by the disciplined, systematic writers. The result of my way is that I do a lot of re-writes, but that would happen anyway, because I end up going in different directions. And when you've got a 400 page novel ms.--ouch!
